METHODS OF PAYMENT
Thank you for choosing Dr. Kevin Gill and Lone Star Spine for your surgical procedure.
Prior to your procedure, we will contact your insurance carrier to obtain the specific benefits for your insurance plan. We are contracted with most managed care insurance plans. Please contact our office for a complete listing. All office visit co-payments and co-insurance are due at the time of visit. We accept all major credit and debit cards as well as your personal check or cash. You may be required to pay a deposit prior to surgery, depending on your particular insurance coverage. You will be contacted by our office to ensure that you are informed of your financial responsibilities and to arrange payment of your portion prior to surgery.
Please be advised that the information that is received from your insurance carrier is an estimate only. They do not calculate the final payment amount until after the actual insurance claim is received. This estimate that is provided to you is for the services of Lone Star Spine only. The hospital, anesthesiologist, radiologist and laboratory will be submitting a claim for their services and this may require additional fees for their services.
As we all know, insurance plans can be very difficult to understand. We encourage you to call your insurance carrier to ask any questions that you may have regarding your particular plan.
